I think companies sometimes forget, like ordinary people do, that they need to plan. For small businesses just starting out, this is the biggest problem along with managing cash flow. For enterprise-level companies I think the problem has been that they forget to match IT investments against a long-term budget. But that is shifting with hybrid IT expenses that can be paid out of operating funds. Plans can shift when they need to, and the change is paid for on a short-term basis. But to get to that point takes a long-term plan, and that's what is missing.
